When I took the System Analysis and Design Methods class in school (the only textbook, I ever kept) our Teacher was a guy named Roy Moore who worked for Shell Oil as an IT Director.  He told us about a project early in his career where some empire builder manager with a little bit of power wanted his department to be the next to use computers.  After a thorough analysis was done, it was determined that, while information came into the group, nothing ever came out.  It just kept getting circulated and reports that nobody ever read were generated.  On his recommendation, the department was shut down and the people were moved into more useful groups.

This happened sometime in the early 1980s.
